Today we reported in at the clinic at 8.30am, and waited for a little while before getting ready to accompany the nurses into the OT. We were briefed that we would be witnessing three operations by Dr. Chen today, two of which were surgeries.

The first operation involved a lady patient who had opted to undergo IVF. Firstly, the patient was placed onto the operating table and sedated. The nurses then proceeded to disinfect her using iodine. Next, the doctor used several instruments to enlarge the patient’s vaginal opening before inserting a mini camera into her womb, which was then projected onto a TV screen. The camera had a long hollow needle attached to it, and using the camera as a guide, Dr. Chen located follicle cells in the patient’s womb and used the needle to penetrate these cells to suck out their contents. The substance which had been sucked out was emptied into a test- tube, which was then passed on to a lab technician nearby, who immediately examined the contents to locate any ovum. Whenever an ovum was discovered, the technician released sperms collected earlier from the patient’s husband and guided them to fertilize the ovum. After the ovum was fertilized, the fertilized product is kept in a test-tube for about three days for it to develop into an embryo, before it is released back into the patient’s body when she returns for another operation.

The second operation concerns another lady patient who was unable to conceive naturally, hence the surgery was performed to check whether her fallopian tubes were blocked by blood clots so that sperm was unable to enter during intercourse. The patient was first sedated and disinfected. After that, Dr. Chen used some sharp metal instruments to pierce a small hole through the patient’s belly button into her body, and a tube was passed through this hole to pump carbon dioxide gas into her body cavity to enlarge it so there will be enough space for manipulation during surgery. Then, the tube was removed and a small camera with a light inserted in its place to illuminate the interior of the patient’s torso. When that was done, a small incision of about 2mm wide was made at one side of the patient’s body and a hollow metal tube was inserted into it to prevent the hole from closing up. This hollow tube served as an entrance for several long thin instruments that the doctor would be using inside the patient later on. Next, the doctor cleaned the patient’s womb of blood clots through the vaginal opening using a long metal rod with a hoop at the end, then pumped blue dye up through the vagina and cervix into the womb and fallopian tubes. As the patient’s fallopian tubes were cleared of blood clots, the blue dye was able to flow all the way up the tubes to the ovaries and into the body cavity, and that was observed through the camera. With that done, the doctor perceived that the patient now has a chance to conceive and the operation was concluded with the doctor sewing the patient up again.

The third and final operation for the day involved yet another lady patient. Her surgery was to remove fibroids which had developed on her uterus wall, causing her to become less fertile. Fibroids are a type of growth that occurs on the uterus and appears like a cream-colored lump adhering to the uterine wall. The patient’s condition was considered severe as she had about 7 or more fibroids growing on her uterus, some of which were about 10cm in length. A procedure similar to that of the second operation was performed to ensure that her fallopian tubes were not blocked. After that, Dr. Chen made 2 incisions at each side of the patient’s torso, each about 10mm in length, for the insertion of metal instruments to aid the doctor. First, a heating device was used to sear and burn away the flesh surrounding the fibroid so that it can be removed easily without unnecessary blood loss, which would be inevitable if the method of cutting the fibroid off was employed instead. Then, needle and thread were inserted into the body cavity and the uterus wall was sewn up over where the fibroid was attached. The process was repeated till all the fibroids were removed. Next, the doctor used a suction device to help suck the removed fibroids out of the body cavity. The patient’s torso was then sewn up and the operation was concluded.

let me tell you...burnt flesh doesn't smell good...seriously...like when the doctor was burning off the flesh you can actually see the flesh turn from red to yellow to charred black while sizzling away the whole time...and you can actually -hear- the sizzling and smell the burnt smell...you know the barbecue kind?...ya about there...and I tell you I'm still feeling abit queasy now from the thought of it...barbecued human flesh anyone?...

Today was my first day into the job shadowing programme. I have been posted to the Christopher Chen Clinic for Reproductive Medicine (CCCRM) located in the Annexe Block of the Gleneagles Hospital. The day started at 8.30am when I reported at the clinic’s lobby area and met Ofelia, who was going to be my job-shadowing partner for the rest of the attachment. We were greeted by Ms. Pauline, who showed us around the clinic and introduced us to Ms. Debbie, a lab technician assistant whom we were going to be attached to for the day.

As a starter, Ms. Debbie gave us a short briefing about the clinic and what it does. Next, she explained her job to us, which consists of centrifuging blood samples to separate out the yellowish blood plasma from the red blood cells, then testing the plasma samples for hormones. After that she allowed us to handle some blood samples, where we had a hands-on experience of removing blood clots from the sample and extracting the plasma for testing.

Then, Mr. Senthil, who was another lab technician, showed us some samples of live spermatozoa that he was examining under a microscope. His job was to count the number of sperms present in a particular sample. First, he had to make a count of the number of motile sperms present, motile meaning sperms that are able to “swim” on their own. Next, with the help of a red stain, he counted the number of viable sperms. Viable sperms are able to fertilize ova and are not stained, whereas unviable sperms were considered dead and are stained red.

After that, Ms. Debbie taught us how to make cotton swabs for use in the operating theatre (OT), and the rest of the morning was spent making such swabs manually in the laboratory.

After lunch, we were posted back to the laboratory, where Ms. Debbie introduced us to the machinery and gadgets that the lab technicians use to process samples. To name a few, we were shown an automated machine that helps to test blood plasma samples to determine the hormones present and its concentration, and tanks of liquid nitrogen that are used to freeze and keep sperm samples. Ms. Debbie also prepared a microscopic slide of live spermatozoa for us to examine.

When that was done, we went to the nurses’ area of the clinic to observe them at their jobs. While we were there, we made conversation to an Indonesian lady patient and were allowed to watch while the nurses took a blood sample from her. After that, we headed off to Dr. Christopher Chen’s office and sat in for one of his consultations with a Dutch couple. The couple had agreed beforehand to conduct in-vitro fertilization (IVF) to help the wife get pregnant, and we were allowed to watch the fertilization procedure and the surgery involving releasing of the fertilized ovum into the patient’s womb, which will take place tomorrow morning.

Lastly, we sat in for a small “operation” for the Indonesian lady patient we met earlier, which involves the doctor inserting an intra-uterine device (IUD) into her womb so that she will not be able to get pregnant for the time being.

muahaha sounds fun right?...of course it does =)...

went to sinhui's blog just now...haha the bibi and baba class photo damn funny can...and it made me realize something...every time in official photos you know you have to do the lineup according to height thing?...I will always endeavour to try to stand next to a good friend then end up kena splitted up...for the class photo me and bibi were trying our best to sit together...and we almost succeeded can!!...cause I was the 4th shortest in class and she was the 5th...but damn sad larh they purposely put lpm in between us so we were split up...then during the invest band photo also...me and leehui were like how desperate to stand together...we even swapped shoes with each other so she got the higher pair of heels...and in the end the person allocated her to one end and me to the other...we were like damn sad larh...so moral of the story: it's fated who you're supposed to stand next to during photo-taking so there's no point trying to stand with your good friend cause it won't work out...

today was a screwed morning...couldn't sleep well at all last night (I shall elaborate on that later and you all will LAUGH I tell you)...anyway got up at around 6.30 and boarded my bus at 7...seriously!!...and somehow I ended up reaching school at 8.30 (?!!)...don't ask me why I don't know...the only probability is that I did stop at thomson plaza to visit the toilet...but still...I don't think I spent THAT long getting that done...and maybe because it was raining and there was this huge jam all the way along thomson road (which makes up about 1/2 my bus journey)...so I reached school half an hour late...fortunately lpm didn't really say anything...just warned that I better not turn up late for OP or else...and said some little comment about my prettiful physics grade...but other than that she's quite sweet today...oh well...good that she ends the year happily...

after she left the classroom the class slacked about for awhile playing frisbee badminton and tennis...and taking photos for our clique...haha oh man we act like we're not going to see each other again for at least 10 years judging by the amount of photos we took...amusing =D...after which we went for lunch at yiling's insistence...so lousy larh she's like the one always want to go out then stupidly go and call her mule to let him know then kena scolded...so stupid can...go anywhere also need to report to him one meh...that nevermind larh then kena scolding everytime somemore...dumbdumb...ya anyway went pastamania to eat and then went shopping around awhile to help sherlene get stuff for her *ahem* date tomorrow...haha so funny...and no her date is not baba =P...

came home and mum dug around my bag for my result slip...and according to her and my dad my grades were "disastrous"...well...I personally thought they were ok really...oh man I seriously should like bring home some other people's grades to show them what "disastrous" really is...maybe something like FOFF...hahaha...sorry...ya as I was saying...BCCO not that bad right?...maybe they were just bowled over by the fact that I didn't have an A...oh well...

yes last night...I went to bed quite late cause I was up doing up this template...so it was like well past midnight...the funny thing was while I lay in bed I kept having the itchy scary feeling that there's someone else in my room with me...you know the funny prickly goosebumpy feeling?...ya and since it was after midnight I had all kinds of funny thoughts about our underground and outerspace friends coming to visit me...freaky can...so finally I fell asleep...then woke up at around 5 something for some unknown reason...and felt this soft warm thing that feels like a hand moving around my throat...oh man I totally freaked larh...flicked it off and it landed somewhere on the floor...and started skittering around on some newspaper I left on the floor...wahlau damn scary larh...so I lay in bed freaking for awhile...then after some time finally mustered enough courage to peer over to the floor...saw this round thing there got more freaked and onned the bedlight...and guess who I found...hamu -_-...haha couldn't describe the relief I felt larh...carried him back to his cage where he immediately dug a hole and flopped to sleep...heh think he must be like damn tired after running around my room the whole night...only thing is I can't figure out how he got into my room in the first place...weird...

whee saturday tomorrow...I like saturdays =)...

hmm...think I sort of left a bad impression on her that day in the library...but oh well...not that she's in my immediate social circle anyway so I shan't bother...

to miss GMB: just because you lead a miserable friendless ostracized life doesn't mean you get the right to make other people's lives miserable friendless and ostracized too...so what if she blogs bad about you...it's her blog with her opinions...and what's the point of a blog anyway if all she blogs about is what you want to read...and not that she's blogging any lies about you...plus the fact that you were so nosey in the first place to secretly search for and read it then blow up in her face when she doesn't blog nice things about you...yea well the truth hurts...blame yourself for making them truths anyway...seriously hope you don't try to bother her anymore...I'm sure she feels betrayed enough...

was deciding what theme to base this template on...actually wanted to make a nice innocent sweet page with a bunny doll theme...you know the cute little bunny that nii carries around all the time?...yes that one...but (not really) sadly I couldn't find a nice picture of the bunny...so I wondered around my gallery abit and came across this pretty thing there =>...so I used it =)...was thinking of naming the site "bold & delicious" after ayu's upcoming single but I thought "strip me" was more appropriate ;)...ok I shall not elaborate in case some people start doubting my innocence...

oh...and one last thing...song playing is "fukisusabu kaze no naka de"...whatever the hell that means...but it's a nice song =)...second ending song for reload gunlock...gotta love it...meaning the song not the show...yes and talking of the show...I am SO pissed with the singaporean anime dubbers!!!...they made my pretty darling goku sound like a freaking 8 year old baby!!!...and goku is at least 18 thank you very much...freak them...
